Getting Ready for Your NDIS Plan Review: What You Should Know

28 October, 2025

If you have an upcoming NDIS plan review or if you’re meeting with your NDIS plan manager for the first time, a little preparation can make a big difference. Taking the time to plan ahead ensures you get the right supports, clarity on what can be funded and confidence in managing your plan effectively.

At Charter Care Services, we believe that being informed and organised empowers you to make the most out of your NDIS plan. Here are a few helpful steps to guide you before your next plan meeting:

  1. Make a “Shopping List” of Supports

Before your meeting, write down the supports, services and equipment you’d like to discuss. Include anything your Allied Health professionals (such as your physiotherapist, occupational therapist or psychologist) have recommended.
Having this list ready will help you clearly communicate your needs and priorities to your planner.

  1. Clarify What Can and Cannot Be Funded

During the meeting, ask your NDIS plan manager to confirm which supports from your list can be funded through your plan and which ones may not be. This step helps avoid surprises later and gives you a clear understanding of what’s covered.

  1. Request Written Confirmation

Always ask your planner to provide written confirmation of the decisions made either in your plan notes or by email. This documentation is valuable if questions arise later about what was agreed upon. Having everything in writing keeps things transparent and easy to track.

  1. Maintain Evidence of Progress

Ensure there is evidence of progress with up-to-date allied health provider reports showing what has happened so far, how supports are linked to goals and what outcomes are achieved or not achieved and why. Share what’s worked, what hasn’t, why, and any changes in your life or disability. Explain what you want to achieve next, and what supports you need to get there (with justification).

Ensure Key Supports Are Clearly Itemised

Maintain up-to-date reports from your allied health providers showing your progress, goals, and outcomes.
Share what’s been effective, what hasn’t, and what changes have occurred in your life or disability. Explain what you want to achieve next and the supports you need to get there. Clear evidence helps justify your requests and ensures your plan reflects your current needs.

Final Tip

Good preparation means less confusion and smoother plan management. The more detail and clarity you have in your plan, the easier it is for everyone involved including your Plan Manager, to help you achieve your goals.
